Position Available - LPN - 12p-8:30p, holiday and every 4th weekend rotation Summary of Position: The Licensed Practical Nurse is responsible for direct patient care. Responsibilities: Special Skills and Requirements: • Shall have a valid driver’s license. • Excellent customer service skills. • Detail oriented with excellent communication, documentation, and organizational skills. • Ability to follow instructions. • Ability to work independently without supervision. • Travel to other sites as needed. Travel may be in inclement weather. • Ability to lift 25 lbs of supplies or equipment. • Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking. • Ability to assist with patient care activities such as ambulation, transfers, and positioning. • Responsibilities of Position: • Provide competent nursing care to assigned patient through skilled symptom management, emotional support, appropriate teaching and timely response to patient and family needs including; • Routine Home care Visits • End of Life/Continuous Care • Referrals • Care Center Assistance • Chart Reviews • Other duties as assigned. • Recognize and report promptly to the registered nurse any changes in the mental and/or physical status of the patient. Reporting should be written and oral when appropriate. • Explain hospice services and benefits in regards to Medicare, Medicaid and Private Insurances and knows when to use Palliative Care Interventions’ and ABN’s. • Obtain and enter orders from Medical Directors and Physicians when deemed appropriate. • Order appropriate Durable Medical Equipment (DME) for patient. • Facilitate continuity of care by coordinating admission of home-care patients to in-patient setting (hospital, nursing home), as needed. • Work evenings and weekends as needed to meet the needs of the patients and families and agency backup schedule • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements. • Maintain clinical competency for assigned responsibilities and procedures. Qualifications: Education and Experience: • Graduation from an approved School of Nursing. • Current LPN licensure in the State of Ohio. • Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing.
