We are looking for compassionate and talented team members who share our vision. The Outreach and Admissions Coordinator assists with overseeing and coordinating all referral and admission functions for MADO Healthcare. The overall objectives are to establish effective referral sources to fully utilize MADO’s service capacity, efficient and effective response to referral partners, build referral source satisfaction and loyalty, thereby positively effecting theutilization of services at MADO Healthcare facilities.     ESSENTIAL RESPONSIBILITIES

    • Ensures that all clients are assessed prior to admission either in-person or via phone
    • Reviews, assesses and recommends referrals for appropriate admissions into the facilities
    • Meets with clients face-to-face to complete assessments for placement consideration
    • Assist clients to acclimate to the center when needed
    • Seeks, cultivates, and forms effective relationships with network referral sources of hospitals, managed care organizations, community programs, and governmental entities to positively effect capacity utilization
    • Assists with data collection of the integrated relationship management database to focus on capturing useful data about referrals, admissions, transition and aftercare of clients for the purposes of effective operation and evaluation
    • Assists the Director in creating all electronic and printed outreach material

    QUALIFICATIONS

    • Master's degree in a human service field
    • LSW, LPC, or license eligible preferred
