    Job Summary Do you want to put a smile on a child's face? Do you want to improve the health of your community? As the Care Coordinator at Ready Set Smile, you will use your excellent communication skills to connect with families of diverse backgrounds and connect them to a dental clinic that can address urgent and nonurgent dental needs. You will become a trained MNSure navigator and help families access health insurance to support their overall wellbeing.

    This position is an excellent opportunity for a candidate that is looking to make a tangible difference in their community while using communication, computer, and organizational skills. All RSS staff spend time in the communities we serve, sharing information about our services and educating people about oral health.

    Responsibilities

    Care Coordination

    Connects with caregivers of children with urgent dental needs, works with the family to get an appointment for care (with a different organization) and to secure the ability to pay for it.

    Builds relationships with intake staff at dental offices to support strong referrals.

    Maintain dental electronic records to track dental care, progress of family outreach, and outcomes. We use NESS and Open Dental. Gather and input data for internal evaluation.

    Insurance Navigation

    Maintain MNsure Navigator status.

    Provide health insurance navigation and application assistance to families receiving care from RSS.

    If the person hired for this position is not a registered MNsure Navigator, they will be expected to complete the training and logistical requirements within the first 30 days of employment.

    Community Outreach

    Attend school and community events (average two a month) to make families aware of our program and give dental referral information.

    Learn the RSS program and be able to teach families the importance of oral health.

    Qualifications

    REQUIRED

    High school diploma or GED

    Fluent in Spanish and English, written and spoken

    Strong computer and data entry skills

    Excellent communication and customer service skills

    Ability to provide high-quality patient care

    Transportation to and from work, and in the middle of the work shift

    Ability to work in-office full time

    PREFERRED

    Lived experience and understanding of the community served by RSS

    Approved MNsure Navigator, completed training already.

    Associate's or Bachelor's degree
