    Salary Range: Annual salary of $90,497.62 - 115,500.46 per year structured within a 6-step scale that accounts for experience and qualifications.

    Location: This position is based in Salinas, California, with the potential for a hybrid work schedule. The specific hybrid arrangement can be discussed and tailored based on individual and team needs and requires work in the Salinas office (at least 50% of the time) as well as regular travel throughout Monterey County and occasional out-of-county travel.

    Schedule: Regular business hours are 8:30 a.m. – 5:00 p.m., Monday through Friday

    OVERVIEW

    First 5 Monterey County is seeking a manager to oversee its Community Care Hub. Through this Hub, First 5 Monterey County partners with local community based organizations to provide early childhood-focused direct services, including home visiting and care coordination via Medi-Cal’s Community Health Worker and Enhanced Care Management member benefits. First 5 Monterey County is becoming a Medi-Cal Community Care Hub, defined by the California Health Care Foundation, as “…entities that centralize administrative functions for Medi-Cal direct service provider organizations that address social drivers of health; this may also include centralized contracting, operational infrastructure, and training.”

    First 5 Monterey County provides funding for and works in partnership with local organizations that serve young children to provide direct services to primarily Medi-Cal families. These organizations are contracted with First 5 Monterey to provide early childhood-focused services by building trusting relationships with families, co-creating shared care plans, conducting screenings and assessments, sharing health educational resources, and collaborating with multi-disciplinary teams to support families to reach their goals related to the health and wellness of their children and family. Through these services, we aim to address social determinants of health, reduce health disparities, make connections to mental and physical health care, and support our families to thrive.

    POSITION SUMMARY

    The Community Care Hub Manager is responsible for the day-to-day operations of the Community Care Hub. This is a full-time position that reports to the director of programs and will work closely with the program administrative assistant and evaluation team to support all functions of the Hub.

    The Community Care Hub Manager will oversee the implementation, management, and evaluation of the Community Care Hub, which involves leading a multidisciplinary team, coordinating service delivery, ensuring program compliance, and driving quality improvement efforts. The Community Care Hub Manager will serve as the primary point of contact for Community Care Hub internal and external stakeholders and will be responsible for the operational success and sustainability of the program. Overall responsibilities include but are not limited to: 1) collaborating with the director of programs on program design, leadership and oversight, 2) managing hub operations, 3) collaborating with the evaluation team on data analysis and quality improvement efforts, 4) leading stakeholder engagement and collaboration, 5) ensuring compliance with applicable regulations, and 6) contributing to and leading team development and support.

    This role requires a strong understanding of Medi-Cal regulations, care management practices, and the ability to develop strategies to support families farthest away from opportunities.

    SKILLS AND EXPERIENCE

    • Minimum 5 years full-time, progressively responsible experience in public or nonprofit organizations, including at least 2 years work involving Medi-Cal contracts and/or allied health contracts. Prior experience as a Community Health Worker, care coordinator, case manager, Peer Support Specialist, Home Visitor, or similar role is a plus. Applicants might possess an advanced degree in a related field such as social work, public health, early childhood development, human services, or public administration. Commensurate work experience will also be considered.
    • Presentation development and delivery, including the ability to translate complex issues to different audiences and deliver clear and succinct reports
    • Project management: ability to manage oneself and the team, prioritize multiple tasks with attention to detail, and work autonomously to meet critical deadlines
    • Contract and budget development, administration and oversight, including the ability to meet programmatic outcomes within defined financial and regulatory parameters
    • Interpersonal relations - especially across race, culture, and class - with empathy, respect, humility, self-awareness, accountability, and transparency
    • Strong oral and written communication skills, including active listening and ability to communicate complex policies, procedures and regulations in an accessible way.
    • Team management: trust-building, clear goal setting, supportive check-in and feedback cycles
    • Group facilitation: collaborative agenda-setting, decision-making, synthesis and follow-through
    • Racial equity and social justice: self-awareness of privilege, implicit bias and current stage in the racial equity journey, knowledge of best practices for advancing racial equity and reducing health disparities and ability to navigate challenging conversations on racial-equity related topics
    • Experience with Electronic Health Records/Care Management software and database programs preferred.

    PREFERRED QUALIFICATIONS:

    • Knowledge of early childhood development, infant/family early childhood mental health, family strengthening (including home visiting and care coordination)
    • Knowledge of Case management/care coordination best practices.
    • Understanding of the Medi-Cal health system landscape.
    • In-depth knowledge of CalAIM ECM, Medi-Cal regulations, and managed care principles preferred.
    • Understanding of adult learning theory and its implications for professional development
    • Familiarity with Monterey County people, economy, geography and political landscape preferred.
    • Lived experience and/or close connection with children and families most impacted by system failures preferred. This includes children and families who are Latinx, Black and African American, Indigenous Mexican, undocumented or have mixed documentation status, impacted by the judicial system, work in agriculture, or have special needs.
    • Principles and practices related to First 5 Monterey County's values, especially Race, Equity, Diversity and Inclusion (REDI); Community Engagement-to-Ownership; Trauma-Informed Care; and Reflective Practice
    • Technology applications such as Microsoft 365, Google Suite, Slack, and Asana (project management tool)
    • Bilingual English and Spanish, preferred. Additional language proficiency in Triqui, Mixteco, or other Indigenous languages spoken in Monterey County preferred.

    AGENCY EXPECTATION:

    • Ability to travel within Monterey County to attend meetings as needed.
    • Documented eligibility to work in the United States.
    • Full-time availability during F5MC’s business hours, which are 8:30 a.m. – 5:00 p.m., Monday through Friday. Flexibility may be required for events or to meet project deadlines, including occasional evening or weekend work. Employees will be expected to be onsite for key meetings and collaborative work while having the flexibility to work from home on other days
