Job Summary:
The role of the ALF & Behavioral Health MA is to gather and write out all the patient information to include; progress notes, lab and x-ray results, and diagnosis for the physician’s review and correction. Communicating appropriate information between physicians and practitioners.
Essential Duties and Responsibilities:
Gather charts and labs in EHR and organize patient information for the physician or NP. · Review any new medications, orders or discontinued orders, nurse’s notes, vital signs, new consults, hospital records, sending scripts to pharmacy. · Review communication between practitioners, families and facilities through on-line portal. · Written communication to patients. · Update the patient census on a daily basis for assisted living facilities to determine when patients were last seen. · Remote monitoring of blood pressure devices and weight scales. · Adheres to all applicable laws and regulations. · Maintains confidentiality within the workplace and within the community. · Commitment and alignment to CFP’s mission, vision and values. · High degree of confidentiality, integrity and professionalism. · Ability to handle protected health information in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). · Performs other related duties as required. This position is subject to change at the discretion of management and additional duties may be applied. · Regularly shows up to work on time according to the established schedule
Education or Experience:
Medical Assistant or equivalent applicable experience preferred
