Evergreen Family Medicine serves outpatient needs, including Urgent Care, Family Practice, Women’s Health, Occupational Health, and school-based telehealth.

    Responsibilities and Duties: • Maintains confidentiality according to HIPPA regulations and EFM policies. • Adheres strictly to EFM departmental standards and policies, including state and federal regulations. • Communicates effectively and professionally with coworkers, managers and patients via phone, email or in person. • Works all tasks in Family Practice New Patient Abstract and Family Practice Abstract ‘buckets’ in an accurate and timely manner. • Advocates for patients by creating appropriate patient cases, requests, communications and appointments in Athena, on paper and in person. • Accurately labels historical medical documents within the EMR. • Keeps an active working knowledge of Athena, engages in continuing education and trainings as offered by manager or company. • Enters all pertinent medical information from health history questionnaires and historical medical records into the patient chart as hard data to facilitate new patient evaluations. • Self Manages to stay on task, maximizes efficiencies and does not distract others as well as encourages others to do the same. • Maintains a positive attitude, does not take work issues personally and does not allow personal issues to affect the work day. • Ensures on a daily basis to promote an environment filled with teamwork, a positive outlook and constant professionalism. • make appropriate call backs for provider • Complete medicine refill requests and fax or call in when needed • Complete patient notification for normal lab & x-ray results • Calling lab, etc. for results

    Qualifications and Skills:

    • High School Diploma or equivalent • Communication, interpersonal, clerical, and organizational skills necessary to complete job duties. • Ability to handle the confidential aspects of the work. • The ability to type at least 40wpm and activate and operate computers and office equipment. • Ability to communicate effectively, both orally and in writing • Set priorities for nursing action in a sequence according to patient needs • Communicate with the provider and other nurses and team members to ensure the patients well being • Teach patient appropriate health information and share written informational material • Maintain a clean, neat and safe work environment for patient and staff according to policy and procedures • Participate in economical utilization of supplies • Minimize non-productive time and fill slow periods with activities that will enable you to prepare to meet the future needs of EFM Education

    Physical requirements:

    • Prolonged periods sitting at a desk and working on a computer. • The employee is frequently required to walk; use hands and fingers, handle, or feel; and reach forward with hands and arms. • The employee is occasionally required to sit and stoop, kneel, or crouch. • May be exposed to patients with infectious diseases. • Must be able to lift up to 20 pounds at times. • Must be able to withstand physical and emotional stress.
