Provide Counseling and Support: Offer emotional support and counseling to patients and their families to help them cope with illness and hospitalization.

Advocate for Patients: Ensure patients' needs and rights are met within the healthcare system, and assist them in navigating the system.

Coordinate Resources: Connect patients with community resources, such as financial assistance, legal aid, housing, and support groups.

Facilitate Discharge Planning: Assist with discharge planning to ensure continuity of care, including arranging post-discharge services and resources.

Crisis Intervention: Provide crisis intervention and emergency support when needed.

Educate Patients and Families: Inform patients and their families about healthcare options, resources, and the patient's condition.

Document Interactions: Keep detailed records of patient interactions, assessments, and care plans.

Ensure that patients receive comprehensive care that addresses both medical and social needs
