Responsibilities include conducting diagnostic evaluations, developing tailored treatment plans with goals and objectives, offering crisis intervention when needed, and following up with patients to ensure they're getting the best care possible. Responsibilities: • Create individualized treatment plans that include goals and objectives for each patient • Evaluate patients’ circumstances on a regular basis and provide necessary follow-up care • Assist with intervention for patients when a crisis occurs • Offer community resources or specialists to patients and family members who are experiencing difficulties • Determine the status of patients mental health by using medical information, interviews, observation, and testing, as well as collaboration with other staff members • Assessing patient needs • Medical social workers assess patients' emotional, physical, and psychological needs to understand what they need and how the social worker can support them. This may include interviewing patients and families to determine their needs • Discharge planning • Medical social workers ensure that patients have a supportive environment to return to and that they have the services they need to function well at home • Developing health care plans • Medical social workers may develop treatment and discharge plans for clients • Educating patients and families • Medical social workers may educate patients and their families • Provide social work services to patients and their families in a medical setting • Conduct assessments to determine the psycho-social needs of patients • Develop and implement care plans based on patient needs and goals • Collaborate with healthcare professionals to coordinate and facilitate patient care • Assist patients and families in accessing community resources and support services • Provide counseling and emotional support to patients and families • Advocate for patients' rights and ensure they receive appropriate care • Document patient interactions and maintain accurate records • Supervise and document limited license medical social workers during counseling sessions • Review and meet with LMSW to discuss their progress Qualifications: • This position requires a high school diploma or GED equivalent and a bachelor’s degree in a field such as counseling or social work; we prefer a master’s degree • Current and unrestricted state license as an LMHC (licensed mental health counselor) or equivalent • A minimum of 1-3 years of experience in a clinical setting working as a mental health therapist • CPR and BLS (Basic Life Support) certifications are required • Exceptional problem-solving and analytical ability as well as outstanding interpersonal and communication skills are needed • Master’s degree in Social Work and active MSW license in [MI] • Minimum of [2] years of social work experience, ideally in-home healthcare or Hospice • CPR certification Compensation: $50 - $65 hourly
