Assistant/Associate Professor, Health Policy
The Area Agency on Aging has openings for two Case Managers.
Both will assess the social service needs of older adults and their family caregivers and arrange services (e.g., minor home repair, homemaker/personal assistance, durable medical equipment, and utility assistance) that meet individual needs and preferences. Such assessments are generally conducted over the phone. The case manager will follow up with all clients at least monthly to ensure that all services were received as authorized and satisfactory in nature.
One case manager will assist older adults with all types of disabilities and their family caregivers and the other will assist informal caregivers of persons with dementia.

Bachelor's degree and 1-3 years related work experience.
