Job Title: Memory Care Coordinator

Job Summary:
The Memory Care Coordinator is responsible for overseeing the development and implementation of individualized care plans for residents with memory impairments, ensuring a high standard of care that enhances the quality of life for residents living with dementia and other cognitive conditions. This leadership role requires a compassionate, organized, and knowledgeable professional who can effectively engage with residents, families, and staff to create a supportive and nurturing environment.
Key Responsibilities:
1. Care Coordination:
- Develop, implement, and regularly review personalized care plans for each resident that address their physical, emotional, and social needs.
- Collaborate with interdisciplinary teams (nurses, caregivers, therapists) to monitor resident progress and adjust care plans as needed.
- Ensure that residents receive appropriate therapeutic activities that support cognitive function and enhance well-being.
2. Staff Training and Development:
- Provide training and support to staff on best practices for memory care, including effective communication techniques, behavioral management strategies, and person-centered care approaches.
- Foster a culture of empathy and understanding toward residents with memory impairments among all team members.
3. Family Engagement and Support:
- Work with families to understand their loved ones' needs and involve them in the care planning process.
- Organize family support meetings and provide resources to help families navigate the challenges associated with memory-related conditions.
4. Program Development:
- Design and implement engaging programs and activities that stimulate cognitive function and encourage social interaction among residents.
- Stay informed about the latest developments in dementia care and incorporate evidence-based practices into program offerings.
5. Compliance and Quality Assurance:
- Ensure all care services meet regulatory and organizational standards for quality and safety.
- Monitor care delivery processes and outcomes through regular assessments and feedback from residents, families, and staff.
6. Community Outreach:
- Represent the memory care program at community events and forums, promoting awareness of memory care issues and available resources.
- Build partnerships with local healthcare providers, organizations, and support groups to enhance services offered to residents and families.
Qualifications:
- Valid state licensure (if applicable) and certifications related to memory care or dementia care (e.g., Alzheimer’s Association Certified Dementia Practitioner).
- Minimum of 2 years of experience in memory care, geriatrics, or related field.
- Strong knowledge of dementia-related illnesses, best practices in person-centered care, and applicable regulations.
- Exceptional interpersonal, communication, and organizational skills.
- Experience in staff training and leadership preferred.
Physical Requirements:
- Must be able to lift, carry, or assist residents as needed.
- Ability to stay on feet for long periods and move throughout the facility.
Working Conditions:
The Memory Care Coordinator will work in a residential setting, requiring interaction with residents, families, healthcare professionals, and other staff members.
Salary: $21-$24 DOE
