    Position Overview

    The School-Based Mental Health Case Manager at Poppy’s Therapeutic Corner provides comprehensive case management and social-emotional skill-building to students facing mental health, social, and behavioral challenges. This role involves assessing student needs, coordinating services, supporting behavioral health skill development, and collaborating with school staff and families to promote academic success and emotional well-being.

    This is a full-time position, with hours during designated school days and hours to align with the school schedule.

    Key Responsibilities

    Assessment & Individualized Support Planning

    Conduct non-clinical assessments of students' mental health, social, and behavioral needs.

    Develop and monitor individualized support plans in collaboration with school staff, families, and external providers.

    Identify barriers to academic success and connect students with appropriate interventions and resources.

    Social-Emotional & Behavioral Health Skill-Building

    Facilitate individual and group sessions to teach coping strategies, emotional regulation, and conflict resolution.

    Support students in developing healthy communication, self-awareness, and stress management skills.

    Implement behavioral interventions and positive reinforcement strategies in coordination with school teams.

    Coordination of Services & Community Partnerships

    Serve as a liaison between students, families, school staff, and mental health providers to ensure coordinated care.

    Connect students and families with community-based mental health services, social supports, and academic resources.

    Advocate for student needs within the school and help families navigate support systems.

    Crisis Intervention & Risk Management

    Provide immediate crisis support for students experiencing emotional distress, behavioral challenges, or mental health crises.

    Develop and implement safety plans in collaboration with school counselors and crisis teams.

    Monitor students at risk for self-harm, substance use, or other safety concerns, ensuring proper interventions are in place.

    Collaboration & Advocacy

    Work closely with teachers, counselors, and administrators to develop strategies that support student well-being.

    Participate in IEP, 504 Plan, and Student Support Team (SST) meetings as needed.

    Educate school staff on trauma-informed practices and social-emotional learning strategies.

    Documentation & Reporting

    Maintain accurate, confidential records of student progress, interventions, and referrals.

    Track service utilization, student outcomes, and program effectiveness to inform best practices.

    Ensure compliance with school policies, HIPAA, and FERPA regulations.

    Qualifications & Skills

    Bachelor’s degree in psychology, social work, counseling, education, or a related field (Master’s preferred).

    Experience in school-based mental health, case management, or behavioral support preferred.

    Strong knowledge of social-emotional learning (SEL), trauma-informed care, and school-based interventions preferred.

    Ability to build trusting relationships with students, families, and school staff.

    Excellent communication, problem-solving, and crisis intervention skills.

    Familiarity with IEPs, 504 Plans, and behavioral support strategies is a plus.
