The QMHP-A is responsible for the provision of direct service care to clients and provides support and assistance to them in the community and home to stabilize current functioning. QMHP-A are responsible for all client related direct service documentation and reports related to the Individual Service Plans. QMHP-A will be responsible for submitting progress notes and discharges prior to client ending services. QMHP-A’s are responsible for ensuring that they receive a copy of the client's ISP to support documentations (Progress Notes). The QMHP-A will assist clients with support to stabilize their mental health symptoms and links to ancillary community services, and advocates for the needs of clients. The QMHP-A works with clients to support their ISP during the approved authorization period authorized by the client’s MCO to support clients' needs during their crisis. QMHP-A participate in weekly supervision/ training to improve and enhance skill level.
EDUCATIONS AND SPECIFICATIONS Minimum Requirement for Qualified Mental Health Professional credentials:
A Bachelor's degree in a human services field.
QMHP A registration
Minimum of 1 year of experience working with clients with mental health conditions
