  • Responsibilities

    We are seeking full-time Licensed Therapists or Qualified Professional to provide assistance and support to mental health consumers in crisis situations; service coordination activities, with or on behalf of a client; psycho-educational and support activities for clients and their families and/or natural supports as appropriate; rehabilitative and independent living skills; and symptom monitoring and management skills. Duties also include the assessment and reassessment of the client’s need for services; informing the client about benefits, community resources, and services; assisting the client in accessing benefits and services; arranging for the client to receive benefits and services; and monitoring the provision of services.

    Compensation: $25

    We are seeking full-time Licensed Therapists or Qualified Professional to provide the following services:

    • The therapist will conduct assessments and provide community-based therapy and case management to adults. May conduct initial assessments for clients with serious and persistent mental health issues and connect them with a higher level of care such as ACT services.
    • Serve as a member of the client’s Community Support Team and take the lead in the development, coordination, revision, monitoring and implementation of the client’s Person-Centered Plan.
    • Assist in identifying, planning, scheduling, monitoring and implementing services for the consumer.
    • Work to coordinate service delivery with other staff and/or across agencies
    • Develop transition plans when clients move in and out of services.
    • Develop therapeutic interventions to work with client.
      Provide any written documentation as required. This will include treatment notes, goals and plans for the medical record (e.g., Service notes, person-centered plan etc.)
    • Be available to respond as needed to clients and co-workers 24/7/365.
    • Must be free from all illicit substance use.

    Direct Services

    • Work to implement behavioral interventions, crisis interventions, and rehabilitative and psycho-educational activities.
    • Work with clients, families, natural supports and collateral agencies in reviewing, applying and achieving goals as indicated in the Person-Centered Plan.
    • Assist and/or coordinate planning and implementing community engagement and other such activities as related to the client’s therapeutic goals and mental health needs.
    • Utilize community-based activities as a means to deliver clinically appropriate services.
    • Provide parent/guardian/natural support training that will aid persons interacting with the client in dealing more effectively with the client.
    • Assist in all service delivery as formally planned with the clinical team.
