Wet Chemistry/Metals Analyst I
Possess a working knowledge base of Home Health coverage criteria, regulatory criteria and Payer source requirements
Possess a knowledge base and ability to apply Medical Center and Home Health policies and processes
Possess a knowledge base and application for the completion of the Patient Comprehensive Assessments at the mandated timeframes of Admission, Recertification, Recertification, transfer or discharge.
From the information obtained from the Comprehensive assessment, Physician initial orders or requested orders, Patient preferences and regulations devises the Patients 60 day Plan of Care meeting expected criteria.
Incorporates and coordinates other Home Health disciplines as appropriate.
Implements the Plan of Care as ordered by the Physician in a systematic approach to meet goals
Coordinates and monitors care/progress of other disciplines involved with the patient
Continually evaluates progress towards goals with necessary adjustments in approach or with Physician orders as indicated.
Follows up on problems identified with Physician notification as appropriate
Has a strong clinical skill mix for all age ranges in the areas of Assessments, Teachings or Interventions.
Has strong teaching / coaching skills using a variety of approved/acceptable media to reach patient goals.
Has the ability to work with and trouble shoot a variety of business i.e computers, cell phones etc and medical equipment i.e IV pumps, wound vacs, feeding pumps etc
Utilizes and Directs to Community Resources
Demonstrates a consistent efficient level of performance in Scheduling/ completing assigned visits and required documentation timeframes and meeting productivity parameters
Demonstrates a consistent efficient level employment requirements and departmental processes.
Maintains consistent timely communications i.e hand off communication, Departmental postings, Management requests and meetings.
Required Skills
MINIMUM EDUCATION: Graduate from an Accredited School of Nursing
Required Experience
MINIMUM WORK EXPERIENCE: Must be able to transport self in inclement weather. Must possess a reliable maintained representative vehicle. At least 1 year experience required. Home Health Experience preferred. Critical Care or Medical / surgical Nursing preferred. Must possess computer / typing skills. Excellent communication skills. Ability to tolerate a high stress environment.
