As a mobile mental health case manager, you will be part of a compassionate team dedicated to providing top-notch care to those in need. Your role will involve working closely with adults with mental health disorders to assess their mental health needs and develop personalized goals and service plans to help them live independently. Responsibilities: • Conduct mental health assessments for clients in various locations, including homes and community centers. • Develop personalized service plans in collaboration with clients to address their mental health needs and goals. • Coordinate with healthcare providers, social services, and community resources to ensure comprehensive care for clients. • Maintain accurate and up-to-date documentation of client progress and treatment plans. • Maintain a productivity rate of at least 55%. Qualifications: • 1-2 years' experience in mental health case management. • Bachelor's degree in psychology, social work, or related field. • Strong interpersonal communication skills. • Knowledge of mental health resources and services in the local area. • Valid driver's license and reliable personal vehicle. Compensation: $23 - $26 hourly
