Immediate need for a Registered Nurse for Mountain Empire Older Citizens Program of All Inclusive Care for the Elderly (PACE). RN needed for patient care for medical clinic visits, triage after-hours calls from patients, make home visits to assess and treat patients. The nurse will be on-call an average of 7 days per month, sharing call with other nurses. The nurse will join a clinical team that includes nurses, one NP and one MD. The medical team is part of a larger interdisciplinary group that works together to care for older adults with chronic illnesses who live at home and need help with activities of daily living.
