    Job Description

    Nurse Practitioner that provides comprehensive primary care and wound care services in Transitional Care Units (TCUs) and Long-Term Care Units (LTCs). The ideal candidate will play a crucial role in ensuring the well-being and health of our residents/patients, providing high-quality care, and collaborating with interdisciplinary healthcare teams. As a Nurse Practitioner (NP) specializing in wound care within long-term care and transitional care units, you will play a crucial role in providing comprehensive healthcare services to patients with complex wounds. Your primary responsibility will be to assess, diagnose, and manage wound-related conditions, collaborating with the interdisciplinary team to develop and implement effective treatment plans. You will work closely with patients, families, and caregivers, ensuring high-quality wound care, promoting healing, and preventing complications.

    Key Responsibilities:

    1. Patient Assessment: Conduct thorough assessments of residents/patients in TCUs and LTCs to establish individual care plans and treatment goals, focusing on their primary care and wound care needs. Perform comprehensive patient histories and physical examinations, considering the patient's overall health status and medical history. Utilize advanced diagnostic skills to identify underlying conditions or complications affecting wound healing.

    2. Primary Care: Provide comprehensive primary care services, including diagnosing, treating, and managing chronic and acute medical conditions, as well as monitoring medication regimens.

    3. Wound Care: Specialize in wound assessment, management, and treatment, utilizing evidence-based practices to promote wound healing and minimize complications. Perform wound care procedures, including wound debridement, irrigation, and dressing changes, ensuring adherence to aseptic techniques and infection control protocols. Monitor wound healing progress and adjust treatment plans as needed, considering patient response and any emerging complications. Collaborate with the healthcare team to manage pain, optimize nutrition, and address other factors influencing wound healing.

    4. Collaboration: Collaborate with the interdisciplinary healthcare team, including physicians, registered nurses, physical therapists, and social workers, to ensure seamless care coordination and a holistic approach to patient care. Consult with other healthcare providers to determine the need for specialized interventions, such as surgical interventions or specialized wound care products.

    5. Care Planning: Develop and implement individualized care plans that address the unique needs and preferences of each resident/patient, with a focus on improving overall health and well-being. Develop evidence-based treatment plans tailored to individual patient needs, considering factors such as wound type, severity, patient preferences, and available resources. Collaborate with the interdisciplinary team, including physicians, nurses, physical therapists, and nutritionists, to coordinate comprehensive care plans.

    6. Medication Management: Prescribe and manage medications, as well as educate residents/patients and their families on medication regimens and potential side effects. Prescribe appropriate wound care interventions, such as dressings, topical agents, offloading devices, and adjunctive therapies.

    7. Patient Education: Provide health education to residents/patients and their families, emphasizing preventive care, lifestyle modifications, and self-management of chronic conditions. Educate patients, families, and caregivers on wound care techniques, prevention strategies, and self-management. Provide emotional support and counseling to patients dealing with chronic wounds, addressing their concerns and promoting self-care.

    8. Documentation: Maintain accurate and up-to-date medical records, ensuring compliance with all regulatory requirements and facility policies. Prepare clear and concise reports summarizing patient progress, treatment outcomes, and recommendations for further care. Adhere to legal and ethical standards in healthcare documentation, ensuring patient privacy and confidentiality.

    9. Quality Improvement: Participate in quality improvement initiatives to enhance the quality of care provided in various settings, contributing to a safe and supportive environment, as well as contributing to the development and implementation of wound care protocols and guidelines to enhance patient outcomes and safety. Stay current with the latest research, evidence-based practices, and advancements in wound care management, incorporating best practices into daily practice.

    10. Emergency Response: Be prepared to respond to medical emergencies promptly, coordinating with the healthcare team and taking appropriate action to ensure resident/patient safety.

    Qualifications:

    - Master's or Doctoral degree in Nursing (NP)

    - Current state Nurse Practitioner license

    - Certification as a Nurse Practitioner in Adult-Gerontology, Family, or another relevant specialty

    - Previous experience in primary care and wound care preferred

    - Strong clinical and assessment skills

    - Excellent communication and interpersonal skills

    - Ability to work collaboratively with a diverse healthcare team

    - Proficiency in electronic health records (EHR) systems

    - Compassion and dedication to delivering patient-centered care
