    POSITION SUMMARY: We have an exciting opportunity to join our team as a CTC Manager. In this role, the successful candidate Provides leadership for Comprehensive Transfer Center (CTC), managing all patient flow activity throughout NYU Langone Health. Manages all patient placement activity for the NYU Langone Health Manhattan campus; including Tisch Hospital, Kimmel Pavilion, Hassenfeld Childrens Hospital, and NYU Langone Orthopedic Hospital. Manages all adult direct admissions, external transfers, and intrafacility transfers into and within NYU Langone Health in collaboration with CTC leadership, CTC Medical Directors, Hospital Chief Medical Officers (CMOs), frontline staff, and members of the interdisciplinary teams. Reports to CTC Director.

    JOB RESPONSIBILITIES:

    • Serves as CTC team lead to ensure strong communication between all team members, external stakeholders and senior leadership. Ensures a collaborative interdisciplinary approach to achieve optional outcomes and further goals of the CTC.
    • Manages all health system Transfer Center and Manhattan Patient Placement & Progression (P3) functions in a responsive manner, utilizing the Electronic Health Record (EHR), approved clinical protocols, and established patient care protocols and healthcare information.
    • Collaborates with and key stakeholders, including Nursing Administration, Unit Dyads, Care Management and Social Work to ensure patient progression occurs in safe and timely manner.
    • Acts as liaison between Transfer Center and key stakeholder departments across the health system, including Patient Access, Revenue Cycle Operations, Regulatory, etc.
    • Serves as CTC team lead for partnership with external Emergency Medical Service (EMS) contracted services.
    • Oversees collection, monitoring, evaluation, and analyzing of departmental data.
    • Models excellent customer service and relationship skills, including in-person, telephone and email communication with key internal and external stakeholders.
    • Manages departmental policies, procedures, and communications to ensure adherence to appropriate processes.
    • Facilitates patient flow throughout the continuum of care, prioritizing placements and transfers by leveraging systematic processing of patient acuity and expert critical thinking and decision-making.
    • Oversees Transfer Center RN Coordinators, ensuring health system transfers and direct admissions take place in safe and timely manner and in accordance with policy. Ensures patient information and clinical status is documented in EHR and is accurately communicated to CTC Medical Directors, accepting attending physicians, nurses and other clinicians.
    • Oversees Manhattan Patient Placement Associates and Pediatric Flow Coordinators to ensure placements take place in a safe and timely manner and an in accordance with approved placement protocols and policies.
    • Identifies and address learning needs of all staff members.
    • Provides leadership for the CTC, serving as a role model and resource for all staff. Provides off-hour support for CTC escalations requiring senior leadership review and/or approval.
    • Other duties as assigned.

    MINIMUM QUALIFICATIONS: To qualify you must have a Bachelors degree. 3-5 years of relevant experience. Current unrestricted New York State RN license. Proficient in EMTALA. Strong problem-solving, multi-tasking, analytical/critical thinking and conflict resolution skills. Ability to establish and maintain collaborative and effective working relationships. Ability to communicate effectively in oral, written and electronic formats (i.e. email, electronic medical record, etc.) Excellent telephone manner and use of professional vocabulary. Ability to function calmly in a stressful situations and fast-paced environment. Positive, enthusiastic, customer-service oriented personality. Proficiency in Microsoft Office suite (Outlook, Word, Excel, etc.).. REQUIRED LICENSES: Registered Nurse License-NYS

    PREFERRED QUALIFICATIONS: BCLS MSN or equivalent degree Epic knowledge Masters Degree in health care related field
