    We are currently looking to expand our management team to better support the needs of seniors in our service area. We are seeking a Registered Nurse with a strong background in home health and/or home care for the elderly. Our ideal candidate is passionate about helping seniors safely stay in their homes as long as possible. They own their results and are driven by excellence.

    Responsibilities (will vary by client):

    Conduct initial and ongoing, comprehensive assessments of the client’s needs and develop a plan of care

    Coordinate total client care by monitoring the client’s condition, promoting sound, preventative practice & coordinating services

    Develop, review & update client care plans

    Answer care plan questions or emerging client concerns & provide guidance as appropriate

    Follow up on client changes, concerns, and incidents in a timely manner

    Communicate with family members as required

    Supervise and train caregivers regarding proper care techniques

    Caregiving as needed

    On-call as needed

    Audit charts for compliance & participate in Quality Improvement Activities

    Be available for on-call shifts

    Set up weekly medication boxes

    Travel throughout seven counties we serve in WV, VA and MD

    Qualifications:

    RN license in MD and/or compact license

    5+ year of practice

    Medical specialty in home health

    Knowledge of MD COMAR

    Strong communication and organization skills

    Experience training and managing caregivers, CNAs, med techs and others

    Ability to adapt to change with positivity and professionalism

    3+ years experience in elder care

    Valid Driver's License and Auto Insurance

    Clean driving record
