    Position Overview:

    We are seeking a qualified and compassionate Nurse to join our care team. This role involves providing direct care, monitoring residents' health and wellness, and collaborating with staff to ensure the highest standard of care. The Nurse will also initiate and lead interventions that improve health outcomes for participants, educate residents and their families, and contribute as a member of a multidisciplinary team.

    Key Responsibilities:

    Resident Care & Health Monitoring:

    Conduct regular health assessments, respond to residents' medical needs, and oversee prescribed medications administration and treatments.

    Track and document residents' health status, updating care plans as necessary.

    Intervention & Education:

    Initiate, engage, assess, monitor, educate, coordinate, and provide interventions for residents and their families to improve care quality and health outcomes.

    Educate resident, families, and caregivers on health, wellness, safety, self-management, medication, self-monitoring, action plans, long-term planning, and accessing care.

    Conduct in-person sessions with residents to provide personalized care and health education.

    Multidisciplinary Collaboration:

    Participate actively in a multidisciplinary team, attending resident meetings, consultations, internal team meetings, and external stakeholder meetings.

    Share relevant health information, including resident medical history, medications, immunizations, dietary needs, and determinants of health.

    Service Planning & Referrals:

    Assist in developing, implementing, and monitoring Individual Service Plans (ISPs) by participating in resident meetings, outreach to resident and caregivers, collaborating with healthcare providers, and making referrals as necessary.

    Maintain regular contact and effective collaboration with community providers to advocate for residents and improve their care.

    Health Compliance & Program Metrics:

    Ensure compliance with required health metrics and monitor measures required by the state.

    Connect directly with resident, families, guardians, and team members to educate and address any barriers to health improvement.

    Documentation & Data Collection:

    Maintain accurate and confidential resident records and documentation.

    Collect, track, and submit resident data to support evaluations, program goals, and performance reviews.

    Staff Support & Training:

    Serve as a resource for staff, colleagues, and external members of a resident’s care team.

    Educate and train residents from diverse backgrounds in health practices, communicable illnesses, and universal precautions annually.

    Health & Safety Compliance:

    Follow policies and procedures as outlined by Hope CSS and comply with the Maine State Board of Nursing's nursing scope of practice.

    Adhere to universal precautions and undergo bloodborne pathogens training, including Hepatitis B, as required for potential exposure.

    Licensure & Driving Requirements:

    Maintain a valid Maine nursing license, a current driver’s license, reliable transportation, and a clean driving record per Hope CSS's policies.

    Qualifications:

    Required: Experience in mental health, residential, or community health settings is preferred.

    Registered Nurse (RN) with a valid state license in Maine.

    CPR and First Aid certification.

    Preferred:

    Strong interpersonal and communication skills with a compassionate, resident-centered approach.

    Ability to respond calmly in emergencies and make informed decisions.

    Familiarity with electronic health record (EHR) systems and Microsoft Office Suite.

    Workdays Monday through Thursday 8am to 4pm
