Job Description
This is a Full Time position at 40 hours per week for our Lake Forest and Grayslake clinics. This role will assist in the navigation of our Gyn Onc patients and perform patient triaging. Schedule is Monday through Friday, day shift.

** Responsibilities:**
- Coordinates and plans physician requested multidisciplinary conferences; tracks scheduled procedures and follows pathology results, coordinates with pathologist and radiology the collection of slides and films related to a diagnosis and monitors outcomes of meetings.
- Interprets and guides patient through follow-up for laboratory, radiology and pathology procedures.
- Follows up on patients with complicated or specialized needs.
- Works with patients in the rehabilitation, recovery period following a cancer diagnosis.
- Serves as a resource for patients and their families while coordinating additional resources as requested.
- Coordinates patient referrals to insurance companies and doctors’ offices.
- Monitors quality of care, physician and patient satisfaction through follow up.
- Teaches about cancer health concerns and provides pertinent lectures as needed.
- Educates patients concerning what can be expected prior to, during and after a cancer diagnosis and treatment procedures.
- Evaluates the patient's physical and emotional recovery after treatment utilizing appropriate resources as needed.
- Develops and implements survivorship care plans to patients with a cancer as they complete treatment.
- Identifies, recommends and continues to establish supportive services to meet patients’ needs.
- Develops programs and provides staff and community education as required; maintains reference library.
- Pursue opportunities for public speaking to promote cancer care services at Northwestern Medicine Lake Forest Hospital.
