About the Role:
Our client is looking for a compassionate and skilled Nurse Practitioner to join their team in Modesto, CA. In this role, you’ll collaborate with Primary Care Physicians, Community-Based Primary Care Physicians, and an interdisciplinary team (IDT) to deliver comprehensive care for our client’s participants. You’ll have the opportunity to perform in-person assessments, coordinate care plans, and provide essential healthcare services to frail and elderly participants, empowering them to thrive in their communities.
What You’ll Do:
- Conduct primary and acute care evaluations for participants in collaboration with other healthcare providers.
- Perform comprehensive in-person assessments, both scheduled and unscheduled, and collaborate with the IDT to develop personalized care plans.
- Manage discharge planning for participants leaving acute or long-term care settings.
- Coordinate participant care in nursing homes, providing regular visits and overseeing ongoing treatment.
- Be an active member of the IDT, working with physicians and other care team members to ensure seamless care and 24-hour delivery when needed.
- Participate in rotating night and weekend call to support participant needs.
- Ensure timely and thorough documentation to support participant care and team collaboration.
Who You Are:

Required Qualifications:
- Graduate of an Accredited School of Nursing with RN State License
- Certification as a Nurse Practitioner with Prescriptive Authority
- Active DEA Registration and eligibility for Medicare and Medi-Cal enrollment
- Reliable transportation for community-based care
Preferred Experience:
- Prior experience in PACE (Program of All-Inclusive Care for the Elderly) or similar settings
- Experience working in managed care and with interdisciplinary teams
