About You
- Heart for service: A passion for and commitment to serving high-risk seniors and frail older adults.
- Specialized experience: Experience working with complex and older populations, with at least two years of experience as an RN caring for medically complex individuals prior to becoming an NP.
- Exceptional communication: Strong interpersonal and communication skills to connect with participants, families, and team members.
- Team player with leadership skills: Ability to work both independently and collaboratively in a team setting.
- Educational background: Master’s degree and graduation from an accredited NP/PA program; Certification as a Nurse Practitioner with prescriptive authority; Graduation from an accredited School of Nursing.
- Licensing and certifications: Active, unrestricted NP license and CPR certification; RN license in the State of California, if applicable.; Valid CA driver’s license with a good driving record, personal transportation, and auto insurance (as required by State law).
- Bilingual skills preferred: Fluency in English and Spanish is highly desirable to better serve our participants.
- Clinical expertise: Current knowledge of chronic care and geriatric issues, with a commitment to best practices in these areas
What You'll Do
- Be a cornerstone of participant care by completing assessments, contributing to care planning, and collaborating on service determination requests.
- Build meaningful relationships as you grow and manage a panel of participants, ensuring their unique needs are met.
- Deliver exceptional care by evaluating, initiating, and adjusting medications, diagnostic studies, and treatments with thoughtful supervision.
- Document with purpose, recording participant interactions, assessments, and next steps in our EMR system within established timeframes.
- Coordinate specialized care by making referrals to specialists and consulting as needed.
- Educate and empower participants by performing history and physicals, teaching preventative care, and facilitating advanced planning discussions.
- Respond with agility by evaluating and treating participants wherever they are—whether at the center, in their home, virtually, or in a skilled nursing facility (SNF).
- Collaborate with the best by working closely with behavioral health, pharmacy, and other team members to ensure holistic participant care.
- Stay proactive by ordering, interpreting, and following up on lab tests and results, performing selected procedures, and escalating abnormalities when needed.
- Support continuity of care by documenting chronic conditions via HCC capture and providing on-call coverage as needed.
- Provide additional support within the scope of your license to ensure participants receive the care they need, when they need it most.
