• Full-time Nurse Practitioner position • Schedule: Monday to Friday, 8:00 am to 4:30 pm - Weekend, holiday, and on-call duties required • Location: Mercer office with potential travel within service area - Possible coverage at inpatient care center - Support Medical Director at EverHeart Hospice Responsibilities: • Guide hospice and palliative care program • Provide medical guidance to physician employees and hospice team • Serve as a liaison among program, local physicians, and healthcare professionals Administer direct medical treatment to patients in homes, hospitals, or nursing homes • Show clinical leadership and excellence in practice Responsibilities: Routine Homecare: • Oversee the implementation of the plan of care for patients/families and in coordination with the patient’s physician, evaluate and change the plan of care as necessary to meet the needs of the patient/family. • Consult with the Medical Director when the plan of care is outside of standardized practice and protocols and specialty consults are needed to manage patient care. • Prescribe medications to the extent delegated and licensed after collaboration with the Medical Director. • Update the interdisciplinary team on changes made to the patient’s Plan of Care. • Collaborate with the Admissions Team and Team Managers concerning admissions and referrals. • Providing services, clinical input, and oversight for all patient treatment plans is consistent with hospice and palliative care philosophy. Participate in all components of hospice as requested, including but not limited to: • Perform “Face to Face” visits as directed. • Perform patient visits at the hospice Inpatient Unit Care Center and home setting. • Provide clinical expertise to the interdisciplinary team regarding diagnosis and medical management of patient/family needs, ensuring that the highest quality of care is provided, and that continuity and comprehensiveness are maintained. Palliative Care: • Collaborate with the attending physician to help palliative care patients to manage and plan for disease progression. • Collaborate with referring physician concerning assessment findings, diagnostic, and therapeutic measures. • Evaluate quality of life and responses to treatments. • Perform Palliative Care consults to assess the patient, provide symptom management, educate regarding the disease process, coordinate providers and end-of-life planning, and link patients to available resources to enhance quality of life. Qualifications: • Candidates must possess a master’s degree in nursing (MSN) from an accredited School of Nursing. • Completion of a nurse practitioner program or certification as a Nurse Practitioner is required. • Applicants should hold a current Nurse Practitioner license in the State of Ohio. • Having current staff privileges at hospitals in the Agency’s service areas is necessary. • Preference is given to candidates with a specialization in the area of Family Practice, Oncology, or Hospice. • Knowledge of non-profit organizations is considered beneficial. • A minimum of five years of nursing experience is required, with a focus on clinical nursing care in oncology, hospice, geriatrics, and intensive care.
