Position Overview:

Qualifications:
- Education: Master’s or Doctoral degree in Nursing (FNP or AGNP focus).
- Licensure: Active New York State NP License; Board Certification required.

- Technical Skills: Proficiency in EHR systems, chronic disease management, and infection control protocols.

Key Responsibilities:
- Conduct comprehensive patient assessments, including medical history reviews and physical exams.
- Develop and update individualized care plans for residents with acute/chronic conditions.
- Monitor and manage infections by adhering to facility protocols.
- Educate patients and families on disease management, prevention, and wellness.
- Collaborate with physicians, nurses, and therapists to ensure coordinated care.
- Maintain accurate documentation of treatments, progress, and care plans.
