UM Upper Chesapeake Health (UM UCH) , a member of the University of Maryland Medical System (UMMS), has a Full-time employed position available for a Nurse Practitioner Program Coordinator to join their reputable, well-established transitional care clinic (TCC) located on the UM Upper Chesapeake Medical Center campus in Bel Air, MD.

Qualifications
** Education & Training** : Master of Science degree with completion of a Nurse Practitioner program required. National certification in the area specialty and licensed by the Maryland Board of Nursing. NPI number, DEA and CDS required. Doctorate of Nursing Practice preferred. BLS required; ACLS preferred.
** Work Orientation & Experience** : Minimum of three (3) years of acute care or related experience required.
