    Your Mission:

    • Deliver exceptional primary care to a dedicated panel of patients, focusing on their unique health goals.
    • Champion the 'treat in place' model, ensuring advanced care planning is seamless and tailored to each individual.
    • Conduct thorough assessments, create personalized care plans, and coordinate with care teams to ensure the best possible outcomes.
    • Play a key role in clinical operations such as prior authorizations, medication reviews, and closing quality care gaps (HEDIS and STARs).
    • Act as a trusted advisor to patients, families, and care teams, ensuring everyone is informed and aligned on care needs and goals.

    What You'll Bring:

    • Unrestricted state licensure as an Advanced Practice Nurse and national board certification.
    • Eligible for Medicare participation.
    • 6+ months of experience as an Advanced Practice Provider (preferably with geriatric, hospice, or palliative care populations).
    • Strong clinical skills and experience in treating chronically ill patients, with a preference for nursing home environments.
    • Tech-savvy with excellent communication skills to collaborate effectively with patients and healthcare teams.
