Nutritionist
Location: Washington, DC
Summary/Objective
The Nutritionist at Patient Centered Services will work collaboratively with healthcare providers to assess, plan, and implement individualized nutritional plans for patients. This role involves offering dietary counseling, education, and support to patients across a variety of health conditions, with the goal of promoting optimal nutrition and overall well-being. The Nutritionist will also be involved in educating patients on disease prevention, managing chronic illnesses, and healthy eating habits.
Essential Functions
Conduct comprehensive nutritional assessments for patients, identifying specific dietary needs based on health conditions, allergies, and lifestyle.
Develop and implement customized nutrition plans tailored to individual patient needs and medical conditions.
Monitor patients’ progress, adjust nutrition plans as necessary, and provide ongoing support and follow-up.
Educate patients and their families on proper nutrition, healthy eating habits, and the role of diet in managing chronic illnesses such as diabetes, cardiovascular diseases, and obesity.
Collaborate with physicians, nurses, and other healthcare providers to create holistic care plans.
Provide nutrition counseling to patients, offering strategies for weight management, improving energy levels, and optimizing overall health.
Create educational materials on nutrition and healthy eating for patient distribution.
Stay current with the latest nutrition research and integrate evidence-based practices into patient care.
Document patient encounters and progress in the electronic medical record (EMR) system.
Participate in interdisciplinary team meetings to discuss patient progress and contribute to comprehensive care plans.
Advocate for healthy eating and nutrition awareness through community outreach programs and events.

Position Type and Expected Hours of Work
Full-time or part-time opportunities are available. Flexibility in working hours may be required to meet patient needs, with some evenings or weekends as necessary.
Required Education and Experience
Bachelor’s degree in Nutrition, Dietetics, or a related field from an accredited institution.
Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certification.
Active state license to practice nutrition in Washington, DC.
Minimum of 2 years of clinical experience in nutrition counseling, preferably in a healthcare or home health setting.
Preferred Education and Experience
Master’s degree in Nutrition or Dietetics.
Experience working with specific populations, such as pediatric, geriatric, or chronically ill patients.
Multilingual abilities (especially English/Spanish) are preferred but not required.
Additional Eligibility Qualifications
Current CPR certification.
Valid driver’s license and reliable transportation for home visits.
