    Job Summary:

    ICLS (Individual Community Living Supports) it's a bundled service that includes six service categories. ICLS services offer assistance and support for people who need reminders, cues, intermittent/moderate supervision, or physical assistance to remain in their own homes. You will assist clients with ambulation and mobility, personal care and hygiene, plan and prepare meals, perform light housekeeping duties that clients can't complete on their own.

    IHS (Individualized Home Support): provide support and/or training for people with mental, physical and developmental disabilities. IHS Professionals help individuals achieve their goals in the areas of community engagement, health and wellness, household management, and adaptive skills.

    PSS (Positive Support Specialist) The primary responsibility of the Positive Support Specialist (PSS)is to assist and respond to clients who engage in challenging behaviors that are impacting their independence and self-sufficiency in the community. The Positive Support Specialist will assist individuals to be more independent in performing essential life skills such as meal preparation, personal hygiene, and navigating the community.

    In all position, you will:

    Review Community Supports and Services Plans (CSSPs) and utilize them as a guide on how to work with your client.

    Collaborate with clients' support team members on ways to assist and care for the client.

    Determine which goals your client would like to accomplish and develop a plan on how to assist them in achieving these goals.

    Drive clients to community outings, stores, and appointments.

    Provide supervision and training to clients on how to complete routine household care and maintenance.

    Assist clients in researching and accessing community resources.

    Assess clients' risks and assist them in ways to mitigate these risks.

    Build skills with clients which could include, but are not limited to teaching clients to cook, meal plan, budget, complete paperwork and manage their appointments.

    Guide and assist clients with their activities of daily living.

    Implement strategies with clients on ways to manage their behaviors

    Qualifications

    All staff must be 18 years of age or older.

    Must have at least 1-2 years of supervised experience working with individuals with disabilities and/or challenging behaviors and/or a degree signifying experience within the field (Psychology, Social Work, Human Services, etc)

    Strong interpersonal skills with the ability to work with a variety of individuals and recognize individual boundaries.

    Reliable transportation, current divers license is required and proof of car insurance.

    Knowledge of local community resources is a plus.

    Pass a Minnesota Department of Human Services Background Study. (Make this smaller same as the others)

    Being comfortable visiting clients in their home and/or in the community.

    Valid driver's license, reliable vehicle
