What you’ll be doing:
Improve or minimize residual physical disabilities of the patient.
Return the individual to optimum and productive living within the patient’s capability.
Periodically participate with all other home care personnel in patient’s care planning.
Provide prescribed occupational therapy and all skilled procedures.
Responsible for initial assessment, plan of care maintenance program development and modifications and reassessments every 30 days.
Consult with physicians regarding change in treatment.

Compensation and Schedule:
$30/hr-$42/hr
Monday through Friday
Weekends as needed
What we’re looking for:
A passion to serve and help others live their best lives possible.
Graduate of an Occupational Therapy curriculum accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and /or certified by successfully completing the National Certification examination, -or-
Eligible for the National Registration Examination of the American Occupational Therapy Association
Currently licensed by the Board of Occupational and Physical Therapy Examiners in the state in which practicing, unless licensure does not apply
Minimum of one year in an acute care setting.
One (1) year experience in Home Health preferred.
