    NOW HIRING an Occupational Therapist in New London County for per diem opportunities in a home care setting. PAID for each completed patient visit (with documentation).

    WORK INDEPENDENTLY · 1-to-1 IN-HOME PATIENT CARE

    RESPONSIBILITIES:

    Perform home visits for patients in a home care setting

    Regularly re-evaluates the therapy needs of our clients; initiates, develops, implements, and makes necessary revisions to our patient's plan of care

    Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in condition

    Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition

    QUALIFICATIONS:

    Current license as an Occupational Therapist in Connecticut

    Valid driver’s license

    Reliable transportation and willing to travel in New London County

    Strong communication and interpersonal skills

    Home health experience including Medicare preferred
