Position Overview
The Occupational Therapist delivers compassionate in-home Occupational Therapy services to Agency patients, working under the guidance of the attending physician. This role also supports Agency staff and collaborates with the physician to provide necessary therapeutic consultations, ensuring each patient’s plan of care is both effective and adaptable.
Key Responsibilities
Patient Assessment and Treatment Planning: Conduct comprehensive assessments of patients’ functional abilities and design tailored, goal-oriented treatment plans to enhance independence and cognitive/physical functioning. All treatment plans are developed in consultation with and approved by the attending physician.
Therapeutic Guidance: Educate and support patients and their families on therapeutic practices, including self-care techniques and creative interventions. Provide training on adaptive techniques to improve day-to-day functioning.
Equipment and Device Management: Train patients and family members on the safe and effective use of orthotic and prosthetic devices. Design and create custom adaptive tools as needed, including instructions for their use.
Progress Monitoring and Plan Adjustment: Regularly re-evaluate patients’ progress to assess treatment effectiveness, revising care plans as needed to meet evolving patient goals.
Family and Caregiver Education: Ensure patients and relevant family members understand the full scope of the treatment plan, empowering them to actively support the patient’s recovery.
Documentation and Reporting: Maintain detailed clinical and progress notes, promptly submitting all required paperwork in alignment with Agency policy. Provide periodic updates to the physician and participate in the decision-making process regarding treatment adjustments.
Coordination of Care: Arrange outpatient Occupational Therapy services if required and provide ongoing consultation to other home health team members on therapeutic techniques, equipment usage, and patient care programs.
Professional Development and Team Collaboration: Continuously improve professional skills through in-service education and participate in team meetings, case conferences, and staff development sessions.
Additional Duties: Complete other assigned responsibilities as directed by the supervisor.
Schedule and Shift:
Work Schedule: Standard work hours with potential on-call requirements if specified by the job description.
Travel
Local Travel: Travel to patient homes within the local area as needed for in-home care.
Qualifications
Required
Education: Bachelor's degree or higher.
Experience: 1+ years in Occupational Medicine and 2+ years in Social Work.
Licensure: Valid Occupational Therapist, Registered (OTR) in Illinois (IL).
Certifications: Possess any of the following:
American Heart Association Basic Life Support (AHA BLS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
American Heart Association Pediatric Advanced Life Support (AHA PALS)
Valid Driver's License (DL)
