Certified Occupational Therapist – Home Health
We are seeking an innovative and dedicated Certified Occupational Therapist in San Antonio, TX to provide high-quality in-home care. Our organization is growing and expanding our team to better serve our patients in Central and South Texas.
Job Summary:
This position is for a registered professional who provides occupational therapy to patients of all age groups. The therapist will plan, implement, and evaluate patient care plans to restore or maintain well-being, assess occupational roles and performance for patients with physical, psycho-social, and cognitive impairments, and provide necessary interventions.
Job Qualifications:
- Education: Graduate of an accredited Occupational Therapy Assistant program with an associate-level degree.
- Licensure: Active state license as an Occupational Therapist Assistant.
- Other Requirements: Valid driver’s license.
Environmental and Working Conditions:
- Work is performed in patients’ homes under various conditions.
- Potential exposure to blood, bodily fluids, and infectious diseases.
- Requires the ability to work a flexible schedule and travel locally.
- Some exposure to unpleasant weather conditions.
Physical and Mental Effort:
- Requires heavy lifting, bending, pulling, pushing, and prolonged standing/walking.
- Must be able to meet patient needs under some stressful conditions.
- Requires hand-eye coordination and manual dexterity.
Essential Functions:
- Provide coordinated occupational therapy to patients under the supervision of a licensed Occupational Therapist.
- Assist patients with range of motion (ROM), strength, coordination, adaptive equipment use, environmental modifications, functional mobility, and safety related to daily living activities.
- Participate in the coordination of care to ensure comprehensive patient support.
- Deliver therapy services as ordered by the physician and in line with the plan of care developed by the Occupational Therapist.
- Apply infection control and standard precautions to prevent contamination and disease transmission.
- Complete timely and accurate documentation of patient progress and care.
- Accept clinical assignments that align with education and competence.
- Use effective interpersonal and communication skills to educate patients and caregivers.
- Ensure compliance with all regulatory and third-party payor requirements through education and support.
- Demonstrate commitment to professional growth and competency through continuing education and participation in in-service training.
- Maintain CPR certification and adhere to all professional practice standards.
