Care Coordination: Plan, coordinate, and provide hospice care in accordance with the patient's plan of care while collaborating with team members and family members to ensure needs are met.
Ongoing Evaluation: Continuously evaluate the effectiveness of hospice services for patients and their families.
Patient Assessments: Perform evaluations for hospice care and conduct informational visits as necessary.
Nursing Process: Utilize the nursing process to deliver professional nursing care.
Consultation: Consult with the attending physician and Hospice Medical Director regarding any changes in patients' conditions that require updates to orders and the plan of care.
Scheduling: Submit a weekly patient visit schedule.
IDG Participation: Engage in Interdisciplinary Group (IDG) meetings and case conferences.
On-Call Duties: Perform on-call duties, including during holidays, in accordance with agency policy.
Patient Education: Educate patients and family members about the dying process, the plan of care, and healthcare decision-making guidance.
Dietary Counseling: Provide dietary advice, considering health issues and dietary changes at the end of life.
Death Visits: Conduct death visits as per agency policy.
Health Teaching: Assess the patient, family, and home environment to identify necessary health teaching.
Plan of Care Development: Develop and implement the initial plan of care and collaborate with IDG members to maintain a comprehensive care plan.
Supervision: Supervise and evaluate the care provided by the Hospice Aide at least once every 14 days and as needed.
LPN Oversight: Guide and supervise LPNs a minimum of every 30 days.
Care Plan Preparation: Prepare care plans for Hospice Aides.
