    Who We Are

    Survivor Healthcare is a telehealth clinic dedicated to improving the quality of life for people with cancer by elevating the standard of care they receive throughout their treatment and survivorship. We bridge the treatment gap between primary care providers and oncologists, offering virtual, evidence-based treatment and support.

    We are seeking an Oncology Patient Navigator who will play a pivotal role in establishing trusting and empathic relationships with patients. You will work alongside a clinical team to provide the highest level of care and assist the clinic's daily operations to ensure an excellent patient experience.

    Job Overview

    The Oncology Patient Navigator will be responsible for a range of patient-oriented tasks, including managing patient referrals, enrolling patients into our supportive care program, scheduling appointments, answering clinic phone calls and emails, and connecting patients to external resources. The Patient Navigator will also work alongside our clinical team to optimize our care, including prioritizing and triaging calls based on patient needs.

    Key Responsibilities

    Referral management

    • Receive and process incoming referrals from healthcare providers and other sources
    • Verify patient eligibility and insurance
    • Contact referring parties for additional information, if necessary

    Patient Enrollment

    • Contact patients referred to Survivor Healthcare and provide information regarding the program, insurance, telemedicine, etc.
    • Assist patients in the enrollment process, ensuring all necessary documentation is completed accurately
    • Answer questions and guide patients during the enrollment process

    Scheduling and Appointment Coordination

    • Schedule telemedicine appointments for patients, taking into account their preferences and availability
    • Handle cancellations, rescheduling, and follow-up with patients as needed

    Referral to External Resources

    • Collaborate with the clinical team to identify and refer patients to relevant external resources, support groups, and community organizations

    Telemedicine Support

    • Provide basic technical support to patients who have questions or encounter issues with our telemedicine platform
    • Troubleshoot and resolve telemedicine-related concerns for patients and clinicians

    Qualifications

    • Associate's or Bachelor's degree (healthcare administration or related field is a plus)
    • Previous experience in healthcare administration or a similar role (oncology experience preferred)
    • Adaptable technology skills; experience with Electronic Medical Records (EMR) software is preferred
    • Strong communication and interpersonal skills
    • Excellent organizational and time management abilities
    • Proficiency in Google Suite
    • Detail-oriented and able to maintain confidentiality
