Job Summary:
We are seeking a friendly, professional, and reliable individual Bilingual (Spanish) Onsite Care Coordinator to work onsite at a partner pharmacy as a liaison between the pharmacy and Medistics, enhancing care for the pharmacy’s patient population. The primary focus is to educate patients on care management services (I.E., CCM, RTM), verify eligibility, enroll them in qualified programs, and complete the initial intake visit (I.E., Annual Wellness Visit, under appropriate supervision). Additionally, you will train patients on the use of smart pill caps and assist with setup. The ideal candidate will have strong interpersonal skills, the ability to communicate effectively about healthcare programs, and a commitment to patient-centered care.
Key Responsibilities
Patient Greeting & Enrollment
Greet patients in a friendly, respectful, and timely manner.
Explain the purpose and benefits of care management programs (CCM, RTM, etc.) to patients.
Verify patient eligibility (including insurance, program criteria) and ensure all enrollment documentation is completed accurately.
Serve as a knowledgeable resource regarding program details and requirements.
Initial Intake & Care Visits
Perform or assist with initial intake visits (I.E., Annual Wellness Visits) under the direction of qualified clinical staff, when permitted by scope of practice and state regulations.
Accurately capture and document patient data in the appropriate EHR or other clinical systems.
Smart Pill Cap Training & Setup (RTM)
Train patients on the use of smart pill caps, ensuring they understand device functionality and expectations for ongoing remote therapy monitoring.
Assist patients in swapping out existing medication caps for smart pill caps, ensuring accuracy and proper labeling.
Troubleshoot basic device issues and coordinate follow-up if further technical support is needed.
Coordination & Patient Flow
Manage patient flow, ensuring smooth coordination between pharmacy staff and Medistics clinical teams.
Handle any patient billing inquiries related to qualified programs, escalating complex issues as appropriate.
Data Collection & Reporting
At end-of-shift, provide management with daily enrollment data, identified onsite trends, and patient feedback to continuously improve processes.
Maintain and meet expectations for daily, weekly, and monthly enrollment targets.
Adhere to HIPAA and other confidentiality protocols in handling all patient data.
Collaboration & Team Support
Support the pharmacy and Medistics teams by communicating program updates, patient concerns, and workflow adjustments.
Maintain a positive and collaborative environment, stepping in to assist colleagues when needed.
Perform other duties as assigned by management.
Patient-Centered Focus
Demonstrate empathy and patience, especially when handling sensitive inquiries or high-stress situations.
Ensure patients feel informed and comfortable throughout their visit and enrollment process.
Resolve patient concerns promptly and courteously, escalating to appropriate staff when necessary.
Qualifications
Education & Certification
Active and valid Certified Pharmacy Technician (CPhT) or Certified/Registered Medical Assistant credential preferred.
High school diploma or equivalent (Associate degree in a related field is a plus).
Experience
Previous experience in a healthcare setting is highly preferred but not required.
Familiarity with chronic care management, remote monitoring devices, or Annual Wellness Visits is highly desirable.
Sales or customer service experience is beneficial, particularly in a clinical or pharmacy environment.

Ability to work both independently and as part of a collaborative team.
Physical & Environmental Demands
Physical Demands
Prolonged periods sitting at a desk, with occasional standing and walking to assist patients or perform administrative tasks.
Handling of light office supplies (generally less than 15 pounds).
Environmental Conditions
Work performed in a professional pharmacy/office environment with standard equipment (computers, phones, fax machines).
Interaction with patients in a healthcare setting, including occasional exposure to medical equipment and supplies.
Moderate noise levels during peak hours; temperature remains stable under normal conditions.
