Summary
The Outreach & Referral Specialist is responsible for developing and implementing outreach plans and providing referral coordination to local individuals, agency partners and the community as a whole regarding health plan and social services, processes, and access. This position coordinates and assists in alignment with healthcare transformation, strategic, and early learning goals by performing the following duties.
Essential Duties
- Develops and implements plans to engage and train cross-sector partners to facilitate connection to family support services.
- Develops and implements outreach plans for underserved children and families, in collaboration with the Early Learning Director, the FamilyCore Leadership Team, and the Early Learning Council.
- Develops and implements networking and professional development opportunities to support the home visiting network.
Job Duties
Communications & Public Relations
- Implements public relations campaign for FamilyCore, as per the recommendations of consultants and the FamilyCore Leadership Team.
- Develops and implements outreach plan to regularly engage and train social service agencies and the faith community in social service supports and referral processes.
- Works closely with the Primarily Care Innovation Specialist to develop pathways to family support services in clinic settings.
- Coordinates regular trainings in family support services for clinicians.
Early Childhood Program Expansion
- Tracks and ensures implementation of FamilyCore strategic plan.
- Develops and implements an outreach plan for all publicly funded preschool and kindergarten transition programs in Yamhill County, with a focus on select priority populations.
- Partners with Early Learning Director to develop a No Wrong Door system for all Early Care & Education programs in the hub region.
- Connects with families applying for Preschool Promise or FamilyCore services and works with Data & Reporting Specialist to ensure processing of applications.
- Budgets, plans, markets and identifies biannual events for Yamhill maternal health community.
- Supports team’s efforts in conducting periodic extensive community assessments to inform expansion of childcare and preschool opportunities.
- Provides ongoing coverage for the Service Integration Specialist, as needed.
Home Visitor Network
- Develops and implements annual professional development plan for home visitor network, in collaboration with the Family CORE Leadership Team.
- Coordinates quarterly professional development opportunities for the home visiting network.
- Promotes collaborative communication among home visiting service providers.
Other Duties
- Supports Early Learning events, as needed.
- Actively participates in weekly Early Learning team meetings and monthly Yamhill CCO staff meetings.
- Participates in networking and professional development opportunities provided by the Oregon Health Authority and the Early Learning Division, as applicable.
- Participates in meetings, webinars, workshops and conferences to share professional knowledge with peers around the state and nationally, as applicable.
- Participates in partner agency advisory councils, as assigned.
- Participates in work review and coaching plans, as necessary.
- Maintains punctual, regular and predictable attendance.

Education & Experience
Required:
- Bachelor’s degree in social work, public health, or related field
OR:
- Associate degree in early childhood education
With:
- Five (5) years of experience in early childhood
- Minimum one (1) year experience performing outreach or event planning.
- Minimum one (1) year experience working with at-risk populations.
OR:
- Any combination of education and experience would qualify candidate for the position.
Preferred:
- Experience in care coordination and/or home visiting.
