About the Role
Our Client is seeking a compassionate and motivated Primary Care Physician to join their interdisciplinary team in Stockton. This role emphasizes relationship-building and quality care, particularly in a value-based environment. The ideal candidate will have experience working with frail, elderly patients, with a strong preference for backgrounds in value-based care, PACE, or primary care.
Key Responsibilities
- Deliver comprehensive primary care and evaluate acute illnesses.
- Conduct in-person assessments, creating care plans with the Interdisciplinary Team (IDT).
- Collaborate with Community-Based Primary Care Physicians and the IDT to ensure coordinated care.
- Supervise 2-3 Nurse Practitioners, overseeing complex cases.
- Perform and coordinate discharge planning for patients in acute or long-term settings.
- Handle call coverage, participating in the rotating schedule.
- Review outcomes data, participate in quality improvement initiatives, and develop best practices.
- Uphold a high level of documentation, especially through Athena, to support the team’s care planning.
Ideal Qualifications
- Completion of an accredited residency program (Internal Medicine, Family Practice, Emergency Medicine, or Geriatrics preferred).
- Board Certified with an active physician license in California.
- State CDS Certification, DEA Registration, and experience with Medicare and Medi-Cal preferred.
- Minimum of 2-3 years of experience, ideally in value-based care, primary care, or SNF settings.
