    Schedule: After Hours and Weekends

    Possible Schedule:

    Monday - 4pm to 9pm Tuesday (off) Wednesday (off) Thursday - 4pm to 9pm Friday - 4pm to 9pm Saturday and Sunday - 8am to 5pm

    every other week schedule:

    Monday - 4pm to 9pm Tuesday - (off) Wednesday - 4pm to 9pm Thursday - 4pm to 9pm Friday - 4pm to 9pm Saturday - 8am to 5pm Sunday (off)

    JOB DESCRIPTION SUMMARY Responsible for managing all aspects of the patient intake process during after-hours and weekends. This includes responding to referral inquiries, coordinating initial patient intake activities, ensuring seamless communication with referral sources and care teams, and managing insurance verification and authorization processes as needed.

    ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

    Responds promptly to after-hours and weekend referral inquiries from patients, families, and referral sources.

    Coordinates all aspects of the patient intake process during assigned shifts, ensuring policies and procedures are followed.

    Ensures compliance with all state, federal, and Joint Commission regulatory requirements during intake operations.

    Facilitates communication between referral sources, patients, families, and the clinical team to ensure smooth transitions to care.

    Verifies insurance information and ensures authorization processes are initiated and completed when required.

    Provides timely updates to the day team about new intakes, pending referrals, and any unresolved issues requiring follow-up.

    Maintains a positive working relationship with referral sources and responds to their needs and concerns promptly.

    Documents all referral and intake activities accurately and ensures appropriate handover to daytime staff.

    Identifies and escalates complex or high-priority issues to the Clinical Supervisor or Administrator as needed.

    Ensures HIPAA compliance and protects the confidentiality of patient information at all times.

    Maintains a working knowledge of community resources and assists referral sources in accessing services not provided by the organization when applicable.

    Participates in regular training and updates to stay current with organizational policies, industry standards, and best practices.

    POSITION QUALIFICATIONS

    An associate’s or bachelor’s degree in healthcare administration, nursing, social work, or a related field is preferred.

    Relevant certifications, such as Certified Home Health Administrator (CHHA) or similar credentials, are beneficial.

    A minimum of two years of experience in a healthcare or home care intake role, with on-call or after-hours experience preferred.

    Strong problem-solving and decision-making skills, particularly in high-pressure or time-sensitive situations.

    Excellent communication and customer service skills, with the ability to build rapport with diverse groups.

    Knowledge of healthcare operations, insurance verification processes, and regulatory requirements.

    Demonstrates autonomy, organization, and the ability to multitask effectively during non-standard working hours.

    Proficient in using electronic medical records (EMR) systems and other relevant technology.
