PRC Baker Places is a non-profit residential treatment agency that has provided the San Francisco community with alternatives to institutional care since 1964. Our direct care professionals provide counseling and case management services to previously homeless adults living with co-occurring diagnoses of chronic mental illness and substance use disorder. All agency services are provided from the philosophy of social rehabilitation which integrates teaching daily living skills in a safe and healthy environment.

Position Overview
Peer navigators are individuals who use their experience of recovery from mental health disorders to support others in the early or later stages of their recovery process along with their experience and knowledge of community resources puts them in a unique position to offer support.
Leland House is a Transitional Residential Care Facility and the goal of treatment is to identify and improve individuals independent living skills in preparation for living in the community as independently as possible.

Primary Duties and Responsibilities
· Engage individuals referred by community service providers to Leland House and introduce availability of program residential services.
· Provide culturally competent and consumer driven social support to reduce emotional health and HIV/AIDS related stressors and stigma.
· Participate in reviews of referrals to the program as appropriate.
· Document any necessary information into Leland House client chart.
· Provide ongoing peer support to individuals at Leland House and work as part of the treatment team in coordinating linkage to community resources if necessary, including healthcare and housing services.
· Escort or accompany Leland House clients to community-based appointments.
· Participate in facilitating Independent Living Skills groups.
· Promote agency mission, purpose, and values in relationships with internal and external partners.
· Performs other duties as assigned.
Minimum Qualifications
· High School of GED
· Prefer completion of a mental health, peer counselor certificate program.
· At least 2-4 years of direct personal lived or supportive experience with the SF behavioral health and HIV/AIDS provider systems.
· Knowledge of HIV/AIDS and potential impacts to client’s life.
· Prefer some peer counseling experience, particularly working with diverse populations and individuals.
· Strong interpersonal skills and ability to work effectively, function independently, and interact professionally with a diverse, multi-cultural, and interdisciplinary staff of all levels.
· Exhibit exemplary customer service, compassion, and care in the execution of all duties and interactions.
