About the Role:

We are seeking a PRN/Part Time On-Call Home Health and Hospice Nurse to join our team at Harmony Home Health & Hospice in Clearfield, UT. This role offers the opportunity to provide compassionate care to patients in the comfort of their own homes, while being part of a supportive and dedicated team.

Responsibilities:

Provide skilled nursing care to patients in their homes

Assess patient needs and develop individualized care plans

Administer medications and treatments

Communicate with physicians and other healthcare professionals

Educate patients and their families on healthcare needs

Requirements:

Active RN license in the state of Utah

Minimum of 1 year of nursing experience, preferably in home health or hospice

Compassionate and caring demeanor

Strong communication and interpersonal skills

Ability to work independently and as part of a team
