Provide information about those drugs and help patients understand the instructions their doctors or other health practitioners provided. >span class="NormalTextRun SCXW214952972 BCX8">provides supervision to pharmacy technicians.
Duties and Responsibilities :
Standard pharmacist duties to include prescription intake, verification and patient counseling.
Advise physicians and other health practitioners on the selection, dosages, interactions, and side effects of medications.
Evaluate appropriateness of prescription orders with consideration for patient safety and legal requirements.
Performs drug utilization reviews (DUR’s) and document interventions.
Monitors the health and progress of patients in response to drug therapy.
Advise patients and answer questions about prescription drugs, including questions about possible side effects and interactions among different drugs.
Provides information and make recommendations about over-the-counter drugs.
Give advice about diet, exercise, or stress management.
Advises patients about durable medical equipment and home healthcare supplies.
Supervise pharmacy technicians to ensure compliance with all standards of care to include data entry and preparation of medications and work plans.
Comply with Avenue 360’s policies and procedures as well as program specific procedures and protocols.
Demonstrate ability to set priorities and work cooperatively with others within and outside the department.
Performs all other duties as assigned by appropriate management.
Education, Experience, Licensure/Certification and Skills/Abilities Related Requirements:
Bachelors of Pharmacy degree from an accredited program by the Accreditation Council for Pharmacy Education (ACPE).
Current BLS/CPR Certification required
Continuing Education and Training Requirements:
Participates in trainings required by the funding source and/or as required by licensure if applicable
