    As a Case Manager, you play a direct role in saving the lives of our patients each day by providing and promoting an environment of long-term recovery. You will partner with the Client and their treatment team to provide recovery-oriented skill teaching and psychoeducation.

    Essential Duties and Responsibilities:

    The PRP case managers are responsible for managing a caseload of children, adolescents, and/or adults. (Depending on preference and experience)

    Implementation of the individual's Psychiatric Rehabilitation Treatment plan.

    Provide care coordination that includes developing daily living skills, increasing community integration, and helping clients meet rehabilitation goals (such as budgeting, medication compliance, housing, etc.)

    Develop and maintain relationships with other providers in the community

    Attend meetings and collaborate with treatment teams.

    Complete documentation in Electronic Health Record to include daily contact notes, assessments, and monthly summaries as required and within the required timeframes.

    Provide group and individual services within the facility, community, and/or client’s home based on their personal preference.

    Option to facilitate psychoeducation and skill building groups.

    Other duties as requested by supervisor.

    Qualifications:

    · High School Diploma or GED. Associate or bachelor's degree in a related field is preferred.

    · At least 1 year of experience working with clients with severe emotional and/or psychiatric problems.

    · Ability to travel up to 90% of the time

    Ability to use a computer, phone, and electronic health record.

    Have a Maryland State driver’s license,

    Have a reliable vehicle and proof of insurance.

    Be comfortable meeting clients in their home and community.

    Strong time management and organizational skills.

    Willingness to travel between program sites, client homes, and community resources as needed.

    Job Type: Contract

    Pay: $875.00 - $3,000.00 per month
