PRP Supervisor/ Rehabilitation Specialist
The PRP Supervisor/ Rehabilitation Specialist will be responsible for assisting with the day to day running of the PRP. Provide group facilitation services, mental health support and rehabilitation services to children and adults referred for services. This candidate will obtain authorization and provide regular written documentation of the client’s progress i.e. TX Plan, progress notes and initial assessments and provide supervision for direct care staff.
Job Duties:
Has overall responsibility for identifying and developing rehabilitation services
Developing and assuring rehabilitation services meet the needs of the individuals served by the program
Provide oversight for the daily program of rehabilitation services
Educate direct care staff on how to document the correlation between rehabilitation activities and the goals identified in the IRP
Conduct training as frequently as necessary to ensure staff ability to perform their job duties
Provide supervision to direct care staff at a minimum of once per week but may be often based on direct care staff needs
Enter Authorizations for consumer participation in program
Responsible for conducting face to face consumer screenings to newly referred consumers
Assist program Director with hiring direct care staff
Other responsibilities as requested by Program Director
Attend trainings on recovery; psychiatric rehabilitation trauma, supported employment practices, substance abuse and cultural competency. Keep apprised of new research and developments in these areas through journals, website and literature
Link individuals to community resources and peer supports to promote community integration
Responsible for developing rehabilitation plans
Ensure all requires documentation is completed in a timely, comprehensive and legible manner.
Consult and collaborate with all team members including therapist, psychiatrist on a regular basis
Participate in quality improvement and data gathering as assigned
Be aware of financial liabilities, coordinate any financial changes with the billing department and assist in securing their party resources when indicated
Assist individual in gaining access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual.
Provide 24 hour/7 days a week emergency phone coverage
Other related duties as assigned.
