Job Summary
We are looking for a PRN Home Health and Hospice CNA to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
Responsibilities
Perform client personal care needs and support other clients and family every-day-needs as supported by the hospice plan of care
Record and report important health information including temperature, heart rate, respiration rate and/or oxygen saturation, and blood pressure as indicated in the plan of care
Communicate effectively and respectfully with a diverse client population and co-workers
Work under the supervision of a nurse:
Promptly communicating any family or client concerns or changes in status
Consulting with a nurse with regard to suggested changes in treatment
Submit required documentation in a timely manner
Qualifications
Current State CNA Certification in good standing
Current CPR with BLS card
Valid driver’s license, at least state minimum auto insurance, and an operational vehicle
Possess or obtain a good understanding of Federal, State, and local laws and regulatory guidelines governing hospice operations
Great organization skills and an ability to work without direct supervision
Ability to remain calm in emergency or crisis situations, and to be close to clients who may be actively dying
Commitment to adhere to professional boundaries
Computer/Technology literacy and experience documenting on an Electronic Medical Record (EMR) system
Physical Requirements
Duties require extensive standing, walking, and sitting
Requires lifting, positioning, pushing, and/or assistance with transferring patients
Requires frequent reaching, stooping, bending, kneeling, and crouching
