    Physical Therapist – Home Health

    We are seeking a Physical Therapist for our Home Health department in the San Antonio markets. Our organization provides high-quality, individualized physical therapy services to patients in the comfort of their homes. We are growing and adding positions to our team!

    Job Summary:

    The ideal candidate will provide patient-centered physical therapy services, creating and implementing treatment plans aimed at improving mobility, alleviating pain, and enhancing the overall quality of life for patients.

    Job Qualifications:

    • Education: Master’s or Doctorate degree in Physical Therapy from an accredited program.
    • Licensure and Credentials: Active state license as a Physical Therapist. Current CPR certification and valid driver’s license.

    Environmental and Working Conditions:

    • Work is performed in patients’ homes under various conditions.
    • Possible exposure to blood, bodily fluids, and infectious diseases.
    • Ability to work a flexible schedule and travel locally.
    • Some exposure to unpleasant weather conditions.

    Physical and Mental Effort:

    • Requires heavy lifting, bending, pulling, pushing, and prolonged standing/walking.
    • Must be able to meet patient needs under some stressful conditions.
    • Requires hand-eye coordination and manual dexterity.

    Essential Functions:

    • Evaluate patients’ physical conditions and develop customized care plans based on their needs, goals, and medical conditions.
    • Deliver physical therapy treatments and interventions to help patients regain function, improve mobility, and manage pain.
    • Educate patients and caregivers on therapeutic exercises, safety techniques, and the use of assistive devices.
    • Monitor and document patient progress, adjusting treatment plans as needed.
    • Collaborate with physicians, nurses, and other healthcare professionals for a coordinated approach to patient care.
    • Maintain accurate and timely documentation in compliance with regulatory standards.
    • Ensure patient safety and maintain professional boundaries in home settings.
    • Supervise the clinical activity of paraprofessionals.

    Job Type:

    • Full-time
    • Pay: $90,000 per annum
