    Key Responsibilities:

    • Assess and evaluate patients' therapeutic, rehabilitative, and functional statuses.
    • Develop and implement comprehensive care plans tailored to individual needs.
    • Evaluate home environments for safety and functionality.
    • Direct and initiate physical therapy treatments and instruct family members or caregivers on aspects of the therapy process.
    • Identify additional needs for home health services and make referrals as necessary.
    • Prepare and submit clinical progress summaries based on goal attainment.
    • Provide in-service education for nursing staff as needed.
    • Supervise Physical Therapy Assistants in accordance with organizational policies and state regulations.

    Qualifications:

    • Bachelor’s in Physical Therapy or higher learning, from an accredited institution.
    • Licensed Physical Therapist in AZ
    • Minimum of one year of relevant experience; community/home health experience is preferred.
    • Bilingual (English/Spanish) is strongly preferred.
    • Excellent verbal and written communication skills with strong organizational abilities.
    • Current CPR certification.
