Compensation: $31.99-$51.09 (DOE) Sign-On Bonus: $2,000k paid after 90 days of employment.
Location: Central Oregon.
Schedule: Monday-Friday 8:00 am-5:00 pm (full-time).
Travel: Local/regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon. Mileage reimbursement is provided.
What You Get To Do:
Perform initial and ongoing psychosocial and emotional assessments of patients/families with a holistic perspective that incorporates identifying support systems, coping mechanisms, and barriers to care.
Utilizes therapeutic techniques to support patients and families in understanding their options and participating in decision-making around their health needs.
Provides emotional support counseling and complex care management services to palliative care patients and families.
Facilitates care coordination and makes appropriate referrals based on identified needs.
Performs advance care planning, including end-of-life conversations, clarifying medical care goals, and discussing hospice care and bills appropriately.
Assists with patient and family education, including health literacy, cognitive abilities, communication patterns, challenges around their medical care, and learning styles.
Engages patients, hospitals, facilities, and the community in education about palliative care and services provided.
Works collaboratively with palliative care providers to ensure appropriate follow-up for patient needs.
Works collaboratively with community resources to support patients and help ensure continuity of care across service lines and settings, advocating for patient-centered care.
Assesses and supports patients and families regarding anticipatory grief and explores family systems/dynamics and coping methods. Evaluate the need for bereavement counseling or appropriate interventions related to complicated grief.
Facilitates appropriate and measurable goals to help patients achieve their desired quality of life.
Required Skills & Qualifications:
Graduate of a Master’s program in Social Work accredited by the Council on Social Work Education
1-3 years of applicable experience in end-of-life care in a healthcare setting.
Valid State of Oregon Clinical Social Worker license (LCSW) for social work applicants
Specialized certification in palliative care social work is preferred.
Ability to pass a background test, drug screen, and TB test.
Valid CPR Certification.
For those working in the field, current automobile insurance, a valid Oregon driver’s license, and reliable transportation are required.
Knowledge of Federal, State, and Local regulations, Medicare Conditions of Participation, and hospice Medicare benefits.
Knowledge of HIPAA privacy rules and regulations.
Knowledge of electronic medical records systems.
Demonstrate excellent organizational skills and ability to prioritize and meet deadlines.
Strong communication and interpersonal skills.
Ability to work independently and collaboratively within an interdisciplinary team.
