Key Responsibilities:
Assessment, planning, implementation, and evaluation of all occupational therapy modalities to patients.
All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
Improves or minimizes residual physical disabilities of the patient.
Returns the individual to optimum and productive level within patient’s capabilities.
Participates with all other home care personnel in patient care planning.
Takes initial history and makes initial evaluation.
Consults with physicians regarding patient’s progress
Participates in quality assessment and performance improvement activities as assigned.
Prepares clinical and progress notes.
Helps develop the plan of care and revise, as necessary.
Consults with family and agency personnel.
Ability to do extensive bending, lifting and standing on a regular basis.
Requirements:
A graduate of an OT Program recognized by the American Occupational Therapy Association or other National accepted Council or Association
An active license to practice Occupational Therapy in MN
A minimum of four (2) years’ experience as a Physical Therapist, home care or geriatric experience a plus
Must have criminal background check
Must have current CPR certification
Must have active driver's license and clear driving record with reliable vehicle
Computer skills.
Ability to exercise initiative and independent judgment
