Your Role & Responsibilities:
Implement individualized physical therapy care plans under the supervision of a licensed PT.
Guide patients through prescribed exercise programs to improve strength, range of motion, and mobility.
Assist patients in gait training using walkers, crutches, braces, and other mobility aids.
Apply therapeutic modalities such as ultrasound, muscle stimulation, hot/cold packs, and massage.
Monitor and document patient progress, reporting significant changes to the PT.
Educate patients and caregivers on home exercise programs to encourage continued progress.
Maintain sterile procedures and ensure therapy equipment is properly cleaned and maintained.
Participate in quality assurance (QA) and in-service training to stay up to date with best practices.
Qualifications:
Licensed Physical Therapist Assistant (PTA) in the state of practice.
Graduate of an accredited PTA program (CAPTE-approved).
Minimum 2 years of experience as a PTA (home health experience preferred).
CPR certification (required).
Ability to lift and transfer patients weighing 150+ lbs.
Strong communication and interpersonal skills to work effectively with patients and families.
Must pass a criminal background check and meet all state compliance requirements.
Comfortable traveling to patient homes (reliable transportation required).
