The primary function of the Patient Advocate is to help patients communicate with their healthcare provider to get the information needed to make decisions about their health care and to provide interpretation when needed. After interactions with patients, the Patient Advocate will bring forward to the clinical team issues for advocacy that may impact health outcomes. Patient advocates may also help patients schedule appointments with specialists, labs or for medical tests, and provide information regarding transportation options if transportation is a barrier to care. Additionally, Patient Advocates may screen patients for Social Determinants of Health (SDOH).
Minimum Qualifications
High school diploma, or equivalent, required
Fluency in Spanish and English required
